General public: MONDAY, JUNE 3rd, WEDNESDAY, JUNE 5th, FRIDAY, JUNE 7th 10:30AM-5PM at BPL Flatbush Branch – 22 Linden Blvd
Seniors Only: TUESDAY, JUNE 4th, THURSDAY, JUNE 6th, 10:30AM-4PM at Zest for Life OAC – 261 E 21st St
The following information is required to complete the application:
- Household Information (All Members)
- Social Security Number (SSN)
- Disability status (If applicable)
- Date of birth
- Contact information
- Family size
- Total annual gross income for the entire household
- Pregnancy status for any household member
- (including due date)
- Criminal and sex offender status for household
- members
- Eviction history and dates (if applicable)
- Reason for applying
- Emergency Contact information
